What is included in an Initial Evaluation?
An initial evaluation includes a complete medical history, lifestyle and social review, assessment of sleep patterns, vision and hearing history, nutrition and fitness overview, and general wellness evaluation. From this evaluation conducted by a Portrait Health Patient Coordinator, our team of specialists will review the findings and design a customized course of further testing and treatment, where necessary. A certificate of medical necessity will be provided to parents, as needed, for school district individual education plans (IEP).
